PATH is seeking candidates with to join our Santa Barbara Interim Housing Program as a Respite Care Case Manager.

ABOUT PATH SANTA BARBARA
PATH Santa Barbara is open 24 hours a day, 365 days a year and is dedicated to assisting homeless individuals achieve greater health, wellness, and self-sufficiency, by helping as many as possible access the services they need to improve their health, find employment and transition to stable housing. Our scattered-site Permanent Supportive Housing (PSH) program provides permanent housing and on-site services to individuals who are currently experienced homelessness.
WHAT IS A RESPITE CARE CASE MANAGER?
Case Managers help Interim Housing guests break the cycle of homelessness by assisting them with the establishment and pursuit of a permanent housing plan through accessing and maintaining necessary support services in the community. Candidates in this position will need to demonstrate basic understanding of the concepts/disease processes most seen in an emergency room and also understand universal precautions, and isolation techniques. Responsibilities of the Case Manager include:
Providing specific care to the Cottage Recuperative Care Program patients including hygiene, nutrition, ambulation or repositioning, elimination and oxygen
Includes assistance with bathing, if necessary
Providing individualized care as is determined by a registered nurse and hospital discharge paperwork
Processing referrals and conducting intake and individualized needs assessments under the supervision and guidance of program management
Collaborating with guests to develop an Individualized Service Plan (ISP) that addresses barriers to obtaining services, housing and healthcare
Communicating with established guests on a weekly basis to review, evaluate and support the completion of ISP plans
Facilitating psychosocial rehabilitation groups for guests as required
Providing on-going case management support such as:
Assistance with benefits establishment
Assistance with obtaining necessary personal identification and records
Employment assistance and linkage
Housing referral, location, advocacy and application
Providing linkage and coordination support for any services necessary to achieve ISP goals
PREFERRED QUALIFICATIONS
An excellent candidate for this role will:
Certified Nursing Assistant (CNA) or comparable certification including an Emergency Medical Technician with bedside experience
Have knowledge of HMIS
MANDATORY REQUIREMENTS
For this role, candidates must:
Be able and willing to work flexible hours which may include evenings or weekends
Have employment eligibility verification
Have or be able and willing to obtain CPR/First Aid training
Provide proof of full COVID-19 vaccination
Successfully complete the following as a condition of hire:

Have reliable transportation
A valid driver’s license
Proof of insurance and ownership for personal vehicles used during work duties
