Overview

POSITION SUMMARY:

The Primary responsibility of this position is to ensure that youth receive caring and professional case management services when housed, at exit, and for follow up aftercare. Other responsibilities include advocacy, referrals and linkages. In addition, the Case Manager is responsible for the development of case plans and the maintenance of all related records, to provide crisis phone coverage and conduct intake and assessments. Must be able to work in a collaborative process and negotiate multidisciplinary systems. This position will work 50% time Wednesday through Friday.

ESSENTIAL FUNCTIONS:

  1. Provide case management to youth while sheltered and during aftercare to ensure linkage to needed services and continued stabilization. 
  2. Develop, coordinate, and track, in conjunction with the Huckleberry House team, case plan with objectives and referrals. 
  3. Monitor progress and changes with the case plan as necessary for aftercare.
  4. Maintain client files and consistent documentation. Ensure records are maintained in compliance with CCL regulations and Confidentiality laws.
  5. Attend and facilitate daily client review. Attend weekly client review and staff meetings, and attend agency meetings, or other meetings as instructed by program management.
  6. Maintain knowledge of community resources for youth and families and conduct outreach as necessary. 
  7. Collaborate and coordinate services with schools and other community providers already engaged with clients and their families. 
  8. Conduct intakes and assessments
  9. Provide phone coverage, which includes crisis counseling, referrals and linkages to youth and their families.
  10. Assure that house rules and procedures are maintained at all times. 
  11. Follow all safety and emergency procedures as outlined in the agency emergency procedures manual and program manual.
  12. Follow all CCL regulations, including those pertaining to medication distribution, meal preparation, and chemical storage.
  13. Represent Huckleberry Youth Programs in the community at meetings, presentations, or appointments as nee
  14. Assist in maintaining a clean and safe facility, including some light cleaning and organizing of work spaces. 
  15. Assist in the duties of Residential Counselors as needed.
  16. Assist program management in maintenance of client data to ensure compliance with various grants. 
  17. Other duties as assigned.
