POSITION SUMMARY:

Huckleberry Youth Programs is seeking a Community Health Manager to oversee the day to day activities of the Community Health Education team located at the Huckleberry Youth Health Center in San Francisco. These activities include the provision of sexual and reproductive health workshops at local schools (including SFUSD) and community based organizations, individual health education sessions, groups. outreach activities, and a peer health education project. The Health Education team is part of a larger site program that includes case managers, therapists, and medical staff from the Department of Public Health.

ESSENTIAL FUNCTIONS:

  1. Manage the day to day operations of the Community Health Education team. The Community Health Education Team provides the following services: sexual and reproductive health workshops in local schools and community based organizations, individual health education sessions, groups, outreach activities, and a peer education project.
  2. Supervise the Community Health Education team. This includes providing individual supervision sessions to each team member weekly, facilitating weekly team meetings, ongoing training/ coaching and professional development plan and activities for each team member.
  3. Recruit and hire members of the Community Health Education Team in conjunction with the Director of Health Services and Director of Human Resources. Conduct initial onboarding training with new hires.
  4. Oversee the research, development, review and modification of curriculum and training modules as needed, in collaboration with Community Health Educators. Ensure that curriculum is up to date and approved by required entities, such as SFUSD.
  5. Ensure all required documentation is completed by the health education team. Review data regularly to monitor progress towards goals. In conjunction with the Director of Health Services, perform program evaluation activities. 
  6. Complete required reports for agency and funders as directed.
  7. Assist in the development of proposals for renewed or new funding in collaboration with the Director of Health Services, Director of Health and Safety, and Director of Development.
  8. Promote team’s workshops, counseling sessions, and peer health education project through outreach to youth, community agencies, City departments, and schools. Look to develop new partnerships for service delivery. Represent agency and team in community convenings as directed by the Director of Health Services.
  9. Monitor quality of services by conducting frequent observation of activities in schools, at Health Center, and in Community.
  10. Develop and maintain relationships with school and agency personnel as necessary to plan and implement peer education groups, workshops, Q groups and any other site based programming
  11. Conduct sexual and reproductive health workshops in schools and community base agencies, as needed, such as when community Health Educators call out sick.
  12. Coordinate scheduling of workshops, outreach activities, groups and individual counseling sessions.
  13. Oversee the Peer Health Education project in conjunction with the Community Health Educator(s) assigned to this project.
  14. Maintain a team manual with policies and procedures, update as needed.
  15. Ensure effective cross collaboration with staff of other teams at the Huckleberry Youth Health Center site.
  16. Coordinate purchasing and expenses with the Director of Health Services. Provide required financial documents.
  17. Participate in weekly supervision with the Director of Health Services. Participate in site meetings and agency meetings: XMT, San Francisco Management, and All Hands, and other meetings as directed by the Director of Health Services.
  18. Other duties as assigned by the Director of Health Services.

  4. Ability to effectively collaborate with internal and external partners. Ensure cross team collaboration at the Health Center site. Ensure the team is engaged and has connections with other parts of the larger agency.
  5. Passionate and experienced in providing sexual and reproductive health education. Ability to develop, update, and modify curriculum as needed.
  6. Ensure team and agency are using best practices and ethical approaches, by staying up to date on current trends.

WORK ENVIRONMENT: This position operates out of the Huckleberry Youth Health Center (HYHC). This role includes driving and the use of standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. HYHC is located in San Francisco and is accessible by public transportation.

PHYSICAL DEMANDS:

The employee must be able to lift and/or carry up to 10 pounds unassisted on an occasional basis. While performing the duties of this job, the employee is regularly required to talk, hear and sit. The employee is occasionally required to stand, walk and use hands to find, handle, or feel objects, tools or controls. The employee is occasionally required to drive a vehicle.

REQUIRED EDUCATION AND EXPERIENCE:

1. Bachelor’s degree in health education, or related field or equivalent training and/or work experience;

2. Minimum two (2) years managing staff or interns, particularly those at the beginning of their careers

3. Minimum two (2) years experience working with youth from diverse backgrounds 

4. Minimum two (2) years experience with group facilitation on topics related to health (preferably with adolescents).

PREFERRED EDUCATION AND EXPERIENCE:

1. Bi-lingual capability (Spanish or Cantonese) strongly preferred.

2. Strong knowledge of best practices in youth health services and youth development theory.

3. Prior experience in curriculum development, project design, implementation and evaluation.

ADDITIONAL ELIGIBILITY QUALIFICATIONS:

1. Recent TB clearance

2. Valid CA driver’s license & clear DMV record

3. Have or will complete first aid/CPR training

4. Fully vaccinated for COVID 19
